What to Bring to Your First Appointment at St. Hope

To determine your eligibility for possible sliding fee discounts and healthcare programs, St. Hope requests that you bring the following information in order to complete the program registration process.

Insured Patients

For insured patients, please bring the following items:

  • 1. Photo ID

    Any type of identification with your picture and name. Common examples include a driver’s license, a state-issued ID, school ID, or work ID.

  • 2. Proof of Insurance

    A copy of your current insurance card.

Self-Pay / Uninsured

For uninsured patients, please bring the following items:

  • 1. Photo ID

    Any type of identification with your picture and name. Common examples include a driver’s license, a state-issued ID, school ID, or work ID.

  • 2. Proof of Address

    – Examples include a utility bill (cable bill, cell phone bill, etc.), voter registration card, a copy of lease agreement or piece of postmarked mail.

  • 3. Proof of Income dated within the last 30 days

    Proof of income documents must show all household income, including earned and unearned income within the last 30 days. 


    a) Examples: Recent paycheck stubs showing the amount paid before any taxes, proof of earned Income – deductions (gross pay) or proof of self-employment, such as logs/records of earnings or a recent W2. If unable to obtain a paycheck stub or if paid in cash or by personal check before tax deduction, a letter from your employer is acceptable, including date, the amount paid, pay frequency and contact information. 


    b) Entitlement/Benefits Income Examples: Social Security statement, Child Support statement, Retiree/Pension/Financial Aid statement, Proof of Unearned Income or Unemployment benefits statement.


  • 4. Letter of Support (for currently unemployed patients)

    If you are currently unemployed and have not received income within the past 30 days, we will accept a letter of support from the person who is helping you meet your financial needs. The letter should include:

    a) Date

    b) The type of assistance you are receiving. If monetary value is given (cash on hand), it must be listed in the letter. We’ll also need the contact information and signature of the person providing you with the assistance. The letter can also be from an organization, church, or agency that is assisting you.

  • 5. Savings/Checking account statement

    Will only be accepted if all the following criteria are met:

    i. You are currently unemployed and have not received any income within the past 30 days.

    ii. No one is helping you meet your financial needs.

    a) You are living off the money in your bank account. A tax return for the most recent year will only be accepted if you are unable to provide any of the documents listed above.
